What is the purpose of the vaccination?

Personal health


Do you take any regular medicines?
Have you had any vaccine in the past 4 weeks?
Have you had a serious reaction to a vaccine, antimalarial or doxycycline before?
Do you or any of your family suffer from epilepsy?
Have you recently undergone radiotherapy, chemotherapy, steroids treatment?
Do you have history of black water fever, blood disorders (thalassemia or sickle cell anaemia), anxiety, depression, heart problem, lung problem, spleen, liver, kidney, taking immunosuppressive or chemotherapy, diabetes, HIV-AIDS or other medical conditions?
Do you have any allergies, particularly to eggs or chicken protein?
Are you pregnant (or planning a pregnancy) or breastfeeding?

Vaccination


Do you require Yellow Fever vaccine with this consultation?
Have you ever reacted to a previous yellow fever vaccine?
Do you have cancer or have you had cancer in the past (even if it was a long time ago)?
Do you have any illness that might affect your immune system?For example, leukaemias, lymphoma, cellular immune deficiencies, chronic lymphoproliferative conditions, or if you have ever received a stem cell transplant.
Are you living with HIV?
Are you taking any medicines (now or within the last year) that affect your immune system? For example, steroids, biological or non-biological immune modulating medicines, treatment following an organ transplant
Are you having chemotherapy or radiotherapy (now or within the last year)?
Have you ever been told that you may have a problem with your thymus gland (includes myasthenia gravis or a thymoma)?
Have you had an operation to remove your thymus gland (thymectomy) for any reason including during cardiac surgery?
Have you ever had open chest surgery?
Are you pregnant or planning a pregnancy?
Are you breast feeding?
Do you have a first-degree family relative (i.e. a blood relative – mother, father, full brother, sister or child) who has had a serious adverse reaction to yellow fever vaccine?
