Treatment Plan: Dose Initiation and Escalation

Mounjaro requires a structured dose titration schedule to optimise weight loss outcomes while minimising side effects. Dose Initiation:
All patients start on the lowest dose of 2.5 mg once weekly.

This starter dose is primarily to allow the body to begin adjusting to the medication’s effects.

Note: 2.5 mg is a sub-therapeutic dose for weight loss (it’s not expected to cause much weight reduction on its own), but it is critical for tolerability. Most side effects, especially gastrointestinal ones, occur as the dose increases. Starting low helps patients cope with any nausea or GI upset​.


Dose Escalation Schedule:
The dose is increased in 2.5 mg increments at minimum 4-week intervals​. A typical titration schedule is as follows:

Is Mounjaro approved for weight loss?
Mounjaro KwikPen is approved for the treatment of adults with type 2 diabetes. It is also authorised for weight management in adults with a BMI of 30kg/m² or above (classified as obesity), and in those with a BMI between 27kg/m² and 30kg/m² (classified as overweight) who have weight-related conditions such as prediabetes, hypertension, dyslipidaemia, or cardiovascular disease.

Who qualifies for tirzepatide?
Adults with a BMI ≥30, or ≥27 with weight-related conditions (diabetes, hypertension, etc.).

Why no form-only consultations?
To meet GPhC requirements and ensure patient safety, all consultations must be interactive – face-to-face or video.

How is the injection given? Weekly, under the skin using a pen device. Training is provided.
