Ventolin/Salbutamol inhaler consultation

I am 18 years of age or over and agree to identity verification checks.

 Salbutamol inhalers do not replace other inhalers and medicines.

 I should take this inhaler ONLY when needed.

  I know what to do in the event of an acute attack of your respiratory condition.

 I will book an urgent review with your GP if you experience any of the following; your respiratory condition has been worse than usual, your respiratory condition has been causing you to wake up at least one night each week over the last 4 weeks, you are using a reliever inhaler more than 3 times per week.

 I will seek immediate medical help in the following circumstances; if you experience severe wheezing, coughing and chest tightness; if your breathlessness affects your ability to eat, speak or sleep; if you feel drowsy; if your lips or fingers turn blue; or if your symptoms are not being controlled by your inhalers.

 My responses are honest and accurate which are necessary for a safe medical assessment.

 I understand the questions asked, have answered them honestly, and that this treatment is for myself only. I understand the side-effects, other treatment options available.

 I consent to the Online Chemist accessing my NHS summary care record (SCR) to process my treatment, if needed.

 I consent to have Online Chemist store my submitted information so they can respond to my inquiry.
